When you arrive at our office for your first visit, we will ask you to fill out the necessary patient forms that cover your personal information, insurance coverage, medical history and dental history. You will compete a thorough oral examination and take a series of x-rays (if necessary). With all the information provided, your provider will develop a treatment plan to achieve the best results possible for your teeth. When finished, you will work with our office manager to discuss payment options (insurance, etc.) and schedule your next visit.
